Beyond telling us the name of the program, please be sure to include a detailed explanation of all expenses, especially any variances from your original budget, or reallocations of funds. If you have not spent all of the grant funds, please explain why.
Response could include, but not limited to, the following information:
Provide metrics to demonstrate impact. Was your project implemented as planned? If not, what deviations occurred and why? What challenges did you encounter and how did you respond to them? What were the biggest successes? What, if any, are your future plans for the project/program?
